Practice Policies

Office Hours

Office Hours are Monday through Thursday, 9:00 am to 5:00 pm, Friday 9:00 am to 12:00 pm.  Closed for lunch from 12:00 pm to 1:00 pm.  The office will be closed for major holidays.  If you have a medical emergency please call 911.

Appointment Policy

Office visits are by appointment only, the minimum age seen is 12. Same-day sick visits are available upon request. If you must cancel or change your appointment, please call at least 24 hours in advance. There is a $50 fee if you do not show up for your appointment. If you are more than 15 minutes late you may be asked to reschedule your appointment. In case of circumstances that are beyond your control, please call as soon as possible to prevent no-show fees. New patients need to arrive at least 30 minutes before their scheduled appointment time to complete paperwork if new patient forms are not completed prior to the appointment.


Prescription Refills

Please request refills at least one week before your medications run out. This will allow time for the review of records and for prior authorizations to be obtained if needed. Antibiotics will not be called in without patients being seen by the nurse practitioner. Narcotics, benzodiazepines, and other scheduled medications will not be prescribed or refilled without an appointment. Refills will be sent during regular office hours. Please check your medications prior to weekends, holidays, and vacations to make sure you have enough medication. Refill requests sent via the portal are preferred over phone calls, as the portal is more efficient than leaving a voicemail.


Hospital Admissions

The hospitalist service is used for admissions at the hospital of your choice. After discharge from the hospital please make sure you schedule a follow-up appointment with me within 1-2 weeks.  It is very important that you bring your medications to this appointment so your medication list can be accurately updated.


Confidentiality

Your medical and personal information is kept strictly confidential. None of your information will be given to anyone without your verbal or written permission. This includes spouses, relatives, friends, and employers, except when required by law. A form will be available, if wanted, to give written permission for the release of information to the persons of your choice.


Insurance

Quality Primary Care will file your insurance claims for you provided you have provided the necessary information. Please be sure to bring your photo ID and insurance card to every appointment. It is your responsibility to make sure your information remains up to date to prevent insurance denials that can result in the patient receiving a bill for the full amount of charges. For your convenience, you can update your information in the portal.


Deductibles, co-payments, and any balance not covered by insurance are the responsibility of the patient. All payments are expected to be paid at the time of service. For any bounced check there is a fee of $30.

Medical Records

Medical records are available by signed request from the patient.  There is a $20 fee for the first 10 pages of printed medical records and $0.25 for every page after.  Electronic medical records are available without charge.
